Make a Confidential Appointment

After requesting an appointment online, we will contact you as soon as possible, usually by the next morning. If you have not heard from us after 24 hours, please contact us. Please note: If you are requesting an appointment on a Sunday for the next day, Monday, we might not have enough time to confirm your appointment. Feel free to come in, however. We can almost always accommodate you. You are, of course, welcome to call us after 8:00 AM to confirm your appointment.

Confidentiality Notice

The information asked online is no different than what you would be asked over the telephone. Your information will be transmitted by secured, encrypted e-mail. Though we make every effort to keep your information secure, it is still possible that the information you provide could be captured by individuals who are engaged in illegal internet observance. By providing and submitting your confidential information, you accept the risk of sending this information by e-mail to Family Planning Associates Medical Group (“FPA”) personnel. When submitting by e-mail, you also agree not to hold Family Planning Associates Medical Group, including its employees or agents, liable for any damages you may incur as a result of the transfer or use of this information.

By current Arizona state law, a 24 hour consent must be completed in person at our office. Please call our office at 602 553-0440 or come in to our office.

To Our Valued Patients,

Please see the below COVID 19 Patient Info:

    • Facial coverings (over the nose and mouth) are required for all staff, patients, visitors, and vendors in our office
    • If you have tested positive for COVID-19 within the last 14 days, had close contact with a positive or suspected positive case, or have COVID-like symptoms, please call us to reschedule your appointment
    • We are screening for COVID-19 symptoms and checking temperatures at the door; if you present with COVID-like symptoms or have a fever of 100.4°F, we will reschedule your appointment
    • Only patients may wait in our office. If you will be sedated, your driver may need to check in with our staff. Minor patients should be accompanied by one parent or guardian
    • If someone else is paying for your services, they will need to bring a photo ID and their card to our Front Desk. This person will also be screened and should wear a facial covering. We can also set up a PayPal invoice prior to your appointment if someone else is paying for your services

Please know that we are taking every precaution to ensure limited chance of exposure. The safety of our patients during office visits is our highest priority. We are constantly referencing the CDC’s recommendations and will implement all precautions necessary.
